Job Summary: Onacare is seeking a Social Worker for its residential facilities, responsible for establishing contact, developing welcome and intervention programs, and managing social procedures. Comprehensive management of the well-being of older adults At Onacare, we are looking for a Social Worker for the Serentill and La Bordeta residential facilities \_Training plan. \_Immediate incorporation. \- Establish initial contact with the older adult, their family, or reference person. Carry out admissions. \- Develop, implement, and evaluate welcome and intervention programs for the older adult and their family. \- Intervene in situations involving loss and conflict experienced by the care recipient, in coordination with other team members. Provide end-of-life companionship. \- Inform the individual and process matters related to guardianship, advance directives, as well as register and uphold the final wishes of those who have expressed them. \- Complete all necessary procedures for the service user regarding dependency status. \- Prepare social reports for users. \- Participate in the Technical Committee (PIAI). * Minimum 6 months’ experience: conducting admissions, managing dependency-related procedures (PIA, PEV, etc.), participation in PIAI. * Bachelor’s Degree in Social Work. * Competencies / Knowledge: Computer literacy (Internet at user level); proficient written and spoken Catalan; interest in working with older adults. * Temporary employment contract (3 months) * Full-time schedule * Gross monthly salary ranging from '1543' to '1800' * Additional information: \_Full-time substitute contract. \_Schedule to be agreed upon with the work centers, Monday to Friday. \_Salary according to the GERCAT collective agreement: 1543.25 gross/month x 14 payments.
