Summary: We are seeking a social worker for a geriatric residence to provide welcoming, follow-up, and social support to residents and their families, interdisciplinary coordination, and administrative management. Safeguarding residents' rights and well-being A geriatric residence located in Bellvís requires hiring a full-time social worker. Welcoming and follow-up of new residents and their families. Social assessment of the service user and identification of social and family needs. Participation in developing the Individualized Care Plan (PAI). Emotional support and guidance to families during the resident's adaptation process and ongoing evolution. Mediation among resident, family, and facility. Processing documentation and administrative procedures (dependency, disability, pensions, PEV, data updates, etc.). Coordination with social services, primary care centers (CAP), hospitals, and other community resources. Preparation of corresponding social reports and records. Coordination with the interdisciplinary team (nursing, psychology, physiotherapy, occupational therapy, management, etc.). Promotion of residents' social integration and participation within the facility. Accompaniment during bereavement processes, hospital admissions, or situations of social vulnerability. Information and counseling regarding social and socio-health resources. Participation in interdisciplinary meetings and case follow-up. Safeguarding residents' rights, well-being, and quality of life.
