We are seeking an experienced, proactive, and socially sensitive Social Worker who wishes to join a committed interdisciplinary team, where they can contribute their expertise both in direct care and in developing the Foundation’s social and community projects. What will your mission be? You will be responsible for promoting social care for service users and their families, ensuring they receive the most appropriate support, accompaniment, and resources throughout their time at the centre. Additionally, you will play a key role in consolidating the Foundation’s project by strengthening collaborative networks with local social and health services. What will your responsibilities be? • Institutional representation and network collaboration • Represent the Foundation before various local social, health, and community stakeholders. • Establish and maintain ongoing relationships with social services, hospitals, primary care centres, EAPs, organizations, and other resources, facilitating coordination and referrals of individuals who may benefit from the Foundation’s services. • Promote awareness of the Foundation’s resources and services among local professionals, encouraging their use. • Identify opportunities for collaboration that contribute to the development of the Foundation’s projects and expand its social impact. • Participate in community coordination forums, conferences, and working groups, representing the Foundation. • Collaborate in outreach, awareness-raising, and institutional promotion activities. Social Work at the Day Centre • Inform, advise, and accompany interested individuals and their families throughout the admission process. • Manage waiting lists and coordinate admission, stay, and discharge procedures. • Ensure optimal occupancy of centre places by monitoring vacancies, coordinating with referring resources, and managing admission processes efficiently. • Prepare social histories and administrative documentation for service users. • Conduct social follow-up of service users and their families, identifying needs and proposing the most appropriate resources. • Draft social reports and manage administrative procedures linked to services. • Coordinate follow-up of public and private placements and maintain communication with the Administration. • Monitor financial aspects of placements jointly with the Administration department. • Safeguard the rights, participation, and well-being of service users. • Actively participate in the development, follow-up, and evaluation of Interdisciplinary Individual Care Plans (PIAI). • Contribute the social perspective to team decision-making. • Facilitate communication among service users, families, and professionals. • Promote continuous improvement initiatives, clinical protocols, and social innovation projects. • Coordinate the Day Centre’s Participation Council. • Participate in the annual report and quality plans of the centre. * Experience: 5 years.
