We are seeking a **Reference Geriatric Caregiver** for our residential center — a committed, approachable individual with an assistive orientation who wishes to support users through comprehensive, personalized care.
**What will be your mission?**
To serve as a reference figure within the care team, ensuring comprehensive and personalized care for users while providing functional support and coordination to the geriatric caregiver team to guarantee care quality, continuity of care, and adherence to established care plans.
Among your responsibilities will be supporting the auxiliary team in organizing daily activities, fostering communication among professionals, and contributing to the continuous improvement of care for residents.
What will be your duties?
* Conduct the initial assessment of the user and collaborate in developing their Care Plan.
* Participate in health promotion and disease prevention initiatives.
* Administer and monitor prescribed medications and treatments according to instructions from the medical team.
* Detect changes in users’ health status and respond in accordance with established protocols.
* Provide care, support, and accompaniment to users and their families, delivering close, high-quality attention.
* Ensure continuity of care through inter-team communication and documentation of relevant information.
* Participate in end-of-life processes, offering companionship and comfort care.
* Coordinate and functionally organize the auxiliary team, distributing tasks and workload.
* Collaborate with the interdisciplinary team by contributing expertise and guidelines specific to the care domain.
* Adapt care plans to meet the individual needs of each user.

What are we looking for?
* Professionals holding qualifications such as: Medium-Level Vocational Training Cycle in Nursing Assistant Care; Medium-Level Vocational Training Cycle in Care for Dependent Persons; Higher-Level Vocational Training Cycle in Social Integration; Professional Certification in Sociosanitary Care for Dependent Persons in Social Institutions; or Professional Certification in Sociosanitary Care for Persons at Home.
* Prior experience in the sector.
* Valid driver’s license and personal vehicle.
