The program aims, on one hand, to support older adults with mental health issues in transitioning to specialized elderly care services. On the other hand, it also seeks to support and involve family members and/or close caregivers, providing assistance and engaging them in the collaborative action plan. **Main Functions and Responsibilities:** * **Individual intervention at the person's home:** Inform the person about the program, its objectives, and intervention methodology. Accompany the person during the intake and program linkage process. Provide individualized follow-up and jointly design personalized action plans with the person receiving support. Evaluate and re-adapt intervention plans. Provide emotional support and accompaniment to help the person advance along mutually agreed life pathways and personal projects.* **Individual intervention in the community:** Familiarize yourself with the operational territory (organizations, services, facilities, etc.) Establish new partnerships with healthcare, social, and cultural services Maintain the person’s connections with key professionals Motivate, foster curiosity, and accompany the person within the community * **Administrative tasks:** Promote the program within the local territory Record follow-up actions conducted with the person Record coordination efforts and referrals to other professionals and local organizations Record and update documentation **Profile:** **PROFILE:** Education: Minimum required qualification is the Social Integration Vocational Training Program.
