Responsibilities: * Conduct pre-admission and admission procedures for service users referred to the center by public administration, informing family members / legal guardians and the center’s team. * Carry out initial assessments of service users in coordination with the interdisciplinary team, and establish objectives within the Care and Life Plan (PAyV) and the Individual Care Plan (PIA), which will include specific activities tailored to each user. * Participate in the evaluation and planning of daily life, leisure, free time, and community integration activities. * Assess the legal, social, employment, educational, and family situation to carry out appropriate interventions. * Inform and advise on existing resources to address identified issues. * Provide guidance and support in processing and managing resources, thereby alleviating the burden on families and/or service users. * Coordinate with public and private agencies for case referrals when necessary. * Inform, advise, and orient individuals, groups, and institutions regarding social action matters. * Prevent the emergence of socially risky situations. * Plan programs and projects promoting prevention, assistance, and development, tailored to the needs of the center’s service users. * Promote the creation, development, and improvement of community resources, initiatives, and social inclusion. * Support the planning, design, and implementation of group activities conducted at the center. * Support individualized care tasks and activities related to the use of and integration into community resources. * Collaborate in community training and awareness-raising activities. Requirements: Mandatory Requirements: Degree in Social Work and specialized training in intervention with minors and families. Highly Valued: Experience working with at-risk families and minors. Knowledge of social resources and the child protection system. Ability to work collaboratively within multidisciplinary teams and coordinate effectively with other professionals; network-based work. Offered: Indefinite contract Salary according to the Social Intervention Collective Agreement. Full-time Morning shifts and at least two afternoon shifts until 8:00 PM.
