Position Summary: We are seeking a professional to participate in the development and follow-up of personalized care plans, comprehensive geriatric assessments, and resident consultations. Key Responsibilities: 1. Participation in a multidisciplinary team and development of personalized care plans 2. Conducting comprehensive geriatric assessments and resident consultations 3. Development of protocols and reports Main Duties: * Collaborate with the multidisciplinary team in developing the Comprehensive Geriatric Assessment, the Personalized Care Plan, and its subsequent monitoring. * Complete the relevant documentation for assigned tasks. * Attend periodic meetings with other members of the multidisciplinary team. * Develop protocols. * Conduct admission assessments: physical dimension including medical history and physical examination. * Assess weight, height, and risk factors. * Conduct nutritional assessment (type of diet and modifications) and health promotion aspects. * Perform pharmacotherapeutic assessment. * Provide clinical consultations to residents. * Prepare reports. Offered: * Permanent contract. * 24 hours per week. * Flexible schedule, preferably mornings. * Salary according to collective agreement.
