We are seeking a social worker in Huelva to provide individualized support to people with chronic kidney disease and their families. Key responsibilities include providing information, guidance, and management of social, healthcare, and administrative resources, as well as preparing social reports and assessing the needs of each case. Social intervention plans are also designed, implemented, and supervised in coordination with hospitals, dialysis centers, social services, and public administrations. Participation includes group activities, health education programs, and awareness-raising initiatives. Furthermore, interventions are documented, and collaboration is provided in the implementation and justification of projects funded by public entities. All work is carried out collaboratively and in coordination with the team. A permanent full-time employment contract is offered, with working hours from 8 a.m. to 3 p.m., and the remainder flexible. The monthly salary is €1,800.
