Experience with elderly people or in the sociosanitary field We are looking for a Social Work professional to join our team on a permanent basis, managing social interventions at our centers in Granollers (Roura) and l'Ametlla (Roig). • Provide information about services and manage admission requests and waiting lists • Plan and coordinate admissions to services • Individual intervention and follow-up • Planning of PAV meetings • Interdisciplinary coordination • Coordination with the community and the network of social services • Compliance with regulations and quality standards * 2 years of experience. Prior experience in social work, preferably with elderly people or in the sociosanitary field. * Bachelor's degree in Social Work * Catalan (spoken: advanced, written: advanced) * Availability of a vehicle * Driving license: B * Permanent employment contract * Full-time position
