Job Summary: We are seeking a social worker to provide counseling, assessment, and coordination of comprehensive support for patients and their families, managing services from initial intake through to case closure. Key Responsibilities: 1. Orientation and counseling in stressful situations. 2. Coordination with various entities for the benefit of the patient. 3. Comprehensive support for patients and their families during sensitive processes. We are looking for a person to perform social worker duties in Pamplona, under a full-time temporary contract covering the vacation period (June to September). Working hours are Monday to Friday, from 09:00 to 16:45\. Duties include handling initial consultations in the office to provide orientation and counseling regarding stressful situations; conducting initial assessments upon admission to develop the social history and social record; and coordinating with municipal governments, social services, sociosanitary centers, hospitals, and other entities for the benefit of patients. The role also involves welcoming new patients upon admission, serving as the primary contact point for both the family and the resident, and supporting the service closure process (due to death or transfer), offering assistance with both bureaucratic procedures and emotional support.
