




Position Summary: Designs and implements individual and family intervention plans, supports socio-labor inclusion, and coordinates with social and health services within an interdisciplinary team. Key Responsibilities: 1. Design and implementation of individual and family intervention plans. 2. Accompaniment in socio-labor inclusion and access to resources. 3. Collaboration within interdisciplinary teams with person-centered care. This position involves designing and implementing individual and family intervention plans, with continuous monitoring of integration processes. Individuals are supported in their socio-labor inclusion, facilitating access to housing, employment, and community resources. Coordination with social services, health services, and other entities is required, including active participation in case meetings. Collaboration within interdisciplinary teams ensures comprehensive and personalized support. Additionally, the role contributes to program planning and improvement, supervising support plans and ensuring their updating and effectiveness. Emotional support and counseling are provided to users and their families, promoting well-being and autonomy. The direct-care team is also planned, organized, and supervised to optimize service quality. Work is conducted collaboratively within interdisciplinary contexts, applying person-centered care models and the social model of disability. Experience in social, health, and community systems—as well as knowledge of available resources in Valladolid and its province—is required. Team management skills and driving ability are valued, although personal vehicle ownership is not mandatory. A minimum of 1 year of experience working with persons with disabilities is required. Experience collaborating in interdisciplinary teams and familiarity with person-centered care models and the social model of disability are essential. Experience in team management or leadership, as well as in social, health, and community support systems, will be considered advantageous. A valid Class B driver’s license and driving experience are required. Immediate incorporation is sought.
