




Position Summary: Designs and implements individual and family intervention plans, coordinating services to support sociolaboral inclusion and well-being of individuals, with a focus on person-centered care. Key Responsibilities: 1. Design and implementation of individual and family intervention plans. 2. Coordination of social, health, and community services. 3. Interdisciplinary team work with person-centered care. The position involves designing and applying individual and family intervention plans, with continuous monitoring of integration processes. Individuals are supported in their sociolaboral inclusion, facilitating access to housing, employment, and community resources. Coordination with social, health, and other agencies is required, including active participation in case meetings. Collaboration within interdisciplinary teams ensures comprehensive and tailored support. Additionally, the role contributes to program planning and improvement, supervising support plans and ensuring their updating and effectiveness. Emotional support and counseling are provided to users and their families, promoting well-being and autonomy. The direct-care team is also planned, organized, and supervised to optimize service quality. Work is carried out collaboratively within interdisciplinary contexts, applying person-centered care models and the social model of disability. Experience in social, health, and community systems is required, as well as knowledge of available resources in Valladolid and its province. Team management skills and driving ability are valued, although personal vehicle ownership is not mandatory. A minimum of 1 year of experience working with persons with disabilities is required. Experience in interdisciplinary team work and familiarity with person-centered care models and the social model of disability are essential. Experience in team management or leadership, as well as in social, health, and community care systems, will be considered an asset. A valid Class B driver’s license and driving experience are required. Immediate incorporation is sought.
