Social Worker \| Coordination Team We are seeking to hire a Social Worker to join our coordination team. We are looking for a candidate with experience in Home Help Services (SAD) and the Dependency Law, capable of working collaboratively and interested in developing their professional activity within a supportive and professional environment. Main responsibilities: * User and family support and follow-up. * Assessment and follow-up of users' needs. * Management and monitoring of services related to SAD and the Dependency Law. * Coordination with auxiliary staff and professionals involved in service delivery. * Resolution and follow-up of incidents. * Support to the coordination team in organizing and monitoring services. * Preparation and monitoring of documentation specific to the Social Work field. * Coordination with family members, professionals, and other resources involved in care. Requirements: * Bachelor's degree or Diploma in Social Work. * Minimum two years of professional experience as a Social Worker. * Experience in SAD, the Dependency Law, and home-based care will be valued. * Strong organizational and management skills. * Excellent communication and interpersonal skills when interacting with users, families, and colleagues. * Ability to work effectively in a team. * Responsible, solution-oriented, and committed to person-centered care.
