Summary: We are seeking a Social Worker to conduct social assessments, design care plans, guide families, and act as a mediator. Highlights: 1. Design and coordination of individualized care plans 2. Guidance to families on available resources and care options 3. Mediation in conflicts among residents, families, or staff Qualifications: BACHELOR'S DEGREE in Social Work or DIPLOMA in Social Work Languages: CATALAN and SPANISH Driving License: B\-CAR \<\= 3\.500 KG, SEATS \<\=9, WITH TRAILER \<\= 750 KG, AND MOTORIZED THREE-WHEELERS AND FOUR-WHEELERS Contract Type: INDEFINITE-TERM EMPLOYMENT CONTRACT Working Hours: Monday to Friday, 09:00–17:00 \-Conduct social assessments of residents to identify their needs. \-Design and coordinate individualized care plans in collaboration with other professionals from the multidisciplinary team. \-Guide families on available resources and care options, as well as on how to support their loved ones. \-Act as a mediator in conflicts that may arise among residents, families, or staff. * Experience: 0 months.
