Within the multidisciplinary team, you will identify the individual’s wishes, preferences, and capacities, as well as their needs, in order to advise them during the acceptance process of support measures. You will also propose a future social approach fostering decision-making aligned with the individual’s will, while ensuring appropriate reception. **What will be your responsibilities?** * Promote and advise on the support measures process and the organization’s functioning for individuals, their family members and/or reference professionals, to achieve effective engagement with prior support. * Develop objectives for the Individualized Support Plan aimed at recovering relevant circumstances identified for the service user, enabling handover to assigned social teams, and achieving effective engagement with their designated support figure. * Manage all administrative procedures related to the acceptance process of support measures up to completion of the initial inventory, coordinating with the individual and other involved professionals. * Collaborate with the rest of the prior support multidisciplinary team to implement the Individualized Support Plan comprehensively. * Perform administrative and documentation tasks required for each acceptance. * Participate with the rest of the team in social area projects or initiatives. **What do we offer?** * Working hours: Full-time * Flexible schedule: You may organize your daily working hours within flexible entry, exit, and lunchtime margins. * Permanent contract * Remote work: 1 day/week * Working hours: Monday to Thursday, 8:00–17:15; Friday, 8:00–15:00 * Intensive schedule (8:00–15:00) during July and August * 24/7 psycho-emotional support hotline, available 365 days per year In compliance with Regulation (EU) 2016/679 on the protection of personal data and applicable legislation, the personal data provided and collected through this job application or CV will be processed and automatically stored in personal data files under the responsibility of Fundació Silvestra Moreno, where such personal data are recorded and archived exclusively for participation in personnel selection processes, job pools, and recruitment activities.
