What will your responsibilities be? * Care process coordination: Act as the patient's primary contact throughout the home respiratory therapy cycle and coordinate care among prescribing services (Pulmonology / Sleep Unit), Primary Care when applicable, and Oximesa’s clinical and technical teams. * TRD patient monitoring: Conduct periodic clinical-functional follow-up of patients undergoing TRD, early detection of clinical incidents, difficulties in device use or therapy adaptation issues, with special attention to chronic, complex, or vulnerable patients. * Management and monitoring of therapeutic adherence: Implement adherence and follow-up programs required under TRD technical specifications, and activate educational reinforcement measures and personalized follow-up to improve adherence. * Management of non-adherent patients: Identify non-adherent patients according to defined criteria; apply specific protocols for managing non-adherent patients; communicate with prescribing services when appropriate. Remote monitoring and telemonitoring: Supervise data from telemonitoring systems for therapies; analyze relevant alerts, incidents, or deviations; coordinate necessary clinical responses per established protocols. * Care for complex and highly dependent patients: High functional dependency, life support, advanced home ventilation, pediatric patients undergoing TRD; coordinate care with hospital teams, caregivers, and other involved professionals. * Health education for patients and caregivers: Reinforce health education during follow-up interactions. * Management of clinical and technical incidents: Centralize and coordinate resolution of clinical and technical incidents related to therapy.
