The program aims to promote the young person’s recovery or stimulation of community ties, the creation of a personalized pathway, and access to professional guidance to assist them during transitional, changing, or loss-related phases—such as adolescence and early youth. **POSITION DEFINITION** This position falls under SM Catalunya’s operational area and reports directly to the Directorate of Services for People. Main Functions and Responsibilities: * **Individual Intervention with the Young Person:** Inform about the program, its objectives, and intervention methodology. Support the welcome process and integration into the program. Provide individualized follow-up and jointly design personalized plans with the service user. Evaluate and re\-adapt intervention plans. Provide emotional support and accompaniment to enable the young person to advance along agreed-upon pathways and life projects.* **Group Intervention with the Young Person:** Design, plan, and implement activities promoting relational spaces with other young people; identify meaningful activities; enhance motivation and interest; facilitate execution of group-chosen meaningful activities; and foster an environment where personal capacities can be strengthened, enabling future participation in other community activities. Continuously evaluate group activities and redefine them as needed. Coordinate with external individuals and/or auxiliary Youth Network staff delivering group activities.* **Family Intervention:** Guide and accompany the young person’s family (the family must understand what their child is experiencing in order to support them in achieving their goals, motivations, concerns… respecting their pace), both individually and in groups. Design, plan, and implement group activities.* **Administrative Functions:** Promote the program within the local territory. Establish collaboration agreements with educational institutions. Coordinate and supervise territorial project evaluation. Prepare documents related to the organization and operation of the Youth Network: reports, innovative programs. Conduct territorial needs assessments and adopt a proactive attitude toward exploring new funding sources. Economic and administrative management responsibility for the program, supported by the administrative department.
