Its objective: enabling people with chronic illnesses to live longer at home, with greater autonomy and quality of life. This project marks a turning point in the sector, as it connects users, hospitals, and health centers within a single digital ecosystem. It is the first initiative to combine these three technologies at such depth and with such transformative purpose. Among the main challenges and initiatives are: * **Predictive algorithms** to anticipate hospital readmission risks using clustering techniques. * **Deployment of 12 robots**, each with distinct support functionalities. * **Sensory telemonitoring** for early-warning systems detecting physical or cognitive decline. * A **caregiver network** (physical and digital) facilitating meetups, scheduled rest periods, and mutual support based on interests and geolocation. * **Co-creation between health and social services**, enabling more integrated and personalized interventions.
