FUNDACIÓ HOSPITAL DE SANT CELONI requires the incorporation of 2 Internal Medicine Physician positions (Ref. 015/2026\). • Comprehensive clinical assessment of complex pathologies or multimorbidity. • Care for frail patients (PCC/MACA). • Management of acute and chronic medical conditions. • Coordination with other specialties to ensure a holistic approach. • Evolutionary follow-up and therapeutic adjustment according to patient needs. • End-of-life care at home. • Participation in protocols and process improvement. * Minimum 1 year of experience. . * Degree in Medicine * Competencies / Knowledge: • Bachelor’s degree in Medicine and Surgery.
