Description
We are seeking a Support Technician for our Legal Capacity Support Service in Decision-Making at Fundació Vella Terra, for the Osona region, Vic. User profiles include mental health conditions, disabilities, behavioral disorders, elderly individuals, etc.
Main responsibilities
* Conduct periodic and follow-up visits to service users at their homes and in community settings.
* Carry out individualized accompaniment tasks in daily management, medical appointments, and administrative or legal procedures.
* Provide support in planning and managing daily life, promoting autonomy and active participation.
* Collaborate in the development, implementation, and monitoring of the Individualized Support Plan (PAIV), tailored to users’ needs and preferences.
* Identify and intervene in situations of vulnerability or risk of abuse, applying protection protocols.
* Facilitate communication and mediation with family members, social networks, and professionals.
* Maintain documentary records and update case files.
* Participate in interdisciplinary team meetings and coordinate with the network of available resources.
Valued professional experience
* Intervention with individuals with mental health disorders in community and residential settings.
* Accompaniment of individuals diagnosed with mental health conditions.
* Accompaniment of dependent elderly individuals.
* Work in the field of substance dependence and addictive behaviors (high-demand programs/harm reduction).
* Support for individuals with intellectual and/or physical disabilities.
* Care and accompaniment of individuals under legal capacity support measures.

Requirements:
\- Vocational Training Cycle in Social Integration
\- Experience in the Decision-Making Support sector.
\- Experience in direct care for individuals with mental health diagnoses or elderly individuals.
\- Knowledge of mental health, disability, and aging.
\- Valid driver’s license and driving experience.
\- Strong motivation to learn.
