HOGAR SÍ Foundation is a non-profit, independent, plural, secular entity not affiliated with any political party. Our primary objective is to permanently and creatively combat social exclusion. To this end, we implement social intervention programs targeting excluded individuals.
**Job Responsibilities**
Reporting to the Technical Coordinator, the main responsibilities are as follows:
* Support the design and implementation of a personalized work plan from a perspective focused on promoting individual autonomy, accompanying the development of strategies to achieve autonomous outcomes in the following core areas:
* Employability and access to economic resources
* Improvement of personal functionality and autonomy
* Promotion of a life project oriented toward autonomy and fostering motivation for change.
* Building social capital, support networks, and community connection.
* Future areas of autonomy development, as well as all actions identified by participating individuals to achieve autonomy.
* Carry out follow-up and support tasks in each housing unit regarding commitments assumed by individuals with the program.
* Ensure provision of basic services and support (personal hygiene assistance, sustenance, basic life skills, etc.) required by individuals.
* Monitor proper execution—and, where applicable, learning—of tasks covering basic needs (hygiene, nutrition, housing care, appliances, etc.)
* Provide and deliver supports and services agreed upon by the participant, adopting a strengths-based perspective through methodologies such as case management, recovery-oriented practice, trauma-informed approaches, and harm reduction.
* Coordinate community-based supports required by the individual to promote their well-being and autonomy from a case management perspective, conducting necessary accompaniments.
· Support and mediate cohabitation relationships among participating individuals—both within shared housing and with neighbors—to ensure fulfillment of commitments undertaken and to promote their autonomy and emancipation from the program.
· Facilitate documentation and supports needed to complete housing-related procedures (residency registration, financial aid applications, access to services, certificates of residence, etc.)
* Train and support individuals in developing strategies for appropriate housing use.
* Accurately and continuously complete intervention, follow-up, and evaluation tools, keeping such records updated, as well as preparing required reports and summaries.
* Participate in supervision and professional practice improvement spaces, as well as supporting practical development and theoretical reflection on innovative methodologies.
* Support the housing management service offered by HOGAR SÍ: coordinate with the entity’s competent department; assist in administrative, legal, or landlord-related processes; support housing adaptation and maintenance; support housing acquisition and return; and generally provide assistance for any other tasks included in the housing cycle.
* Attend coordination meetings, technical forums, and workshops as required.
* And any other additional duties necessary both in the area of housing management and those required in supporting individuals to achieve their autonomy.
**Required Profile**
* Experience: Minimum two years’ professional experience working with homeless individuals in any type of service or support program.
* Desirable: Experience in mental health, mediation, person-centered planning, harm reduction, deinstitutionalization processes, etc.
* Computer literacy at user level or higher (Windows environment)
