Description
The Foundation for the Care of Dependent Persons seeks to incorporate a Social Worker into its team located in Cornellà de Llobregat, **to temporarily cover a maternity leave replacement**.
The selected candidate will join a stable, interdisciplinary, and versatile team, primarily carrying out *functions related to the Home Care Service (SAD), Day Center, and Social Dining Room at the Teresa Duran Residence*.
The incorporation is scheduled for September, with the aim of conducting an onboarding and handover period with the current professional holding the position, thereby facilitating familiarity with the services, service users, families, and internal operations of the Foundation prior to assuming full coverage of the leave.
The responsibilities to be performed are those typical of a Social Worker in the field of care for older adults and dependent persons. Among other duties:
* Coordinate and manage various services related to person-centered care: Home Care Service (SAD), Day Center, and Social Dining Room.
* Lead and conduct informative and orientation visits regarding available services for service users and their families.
* Monitor individual cases and manage incidents related to assigned services.
* Ensure continuity of care throughout the admission, stay, and service termination processes, performing necessary technical and social interventions.
* Serve as the reference professional for direct-care staff associated with the various services.
* Collaborate closely with other professionals to ensure comprehensive, person-centered care.
* Participate in follow-up and coordination meetings with management and interdisciplinary teams.
* Prepare technical documentation, reports, and activity summaries related to service operations.
* Participate in internal training activities and support the mentoring of trainees and students on practicum placements.
* Provide support to other social area services of the Foundation when required, promoting a cross-sectional perspective and teamwork.
Although this position will be primarily linked to these services, the selected candidate will also have the opportunity to learn about and support other social area services of the Foundation, fostering a holistic view of care and collaborative work.
A *minimum of 6 months’ experience as a Social Worker* is required; experience in the *socio-healthcare, residential, elderly care, or dependent-person care sectors* will be especially valued.
A **full-time schedule** is offered, Monday to Friday, with hours from 8:30 a.m. to 5:30 p.m. Monday through Thursday, and 8:30 a.m. to 2:00 p.m. on Friday.
Salary: €14,652.00–€31,488.29 per year
