The Germà Tomàs Canet Foundation, part of the Hospitaller Order of Saint John of God, is a non-profit organization whose mission is to support individuals in situations of vulnerability and at risk of social exclusion through the management of programs and services aimed at improving quality of life and social integration. Currently, we are seeking a Social Worker for the Refugee Support Program. The mission of this position is to accompany individuals in situations of vulnerability—particularly asylum seekers or beneficiaries of international protection—with the aim of facilitating their social inclusion, personal autonomy, and access to resources and rights, promoting integration pathways and contributing to the improvement of their living conditions, in accordance with the values of the Hospitaller Order of Saint John of God.
Reporting to the Coordination team, responsibilities include:
General Functions:
- Provide social accompaniment to individuals served throughout their integration process.
- Conduct welcome and follow-up interviews.
- Carry out social diagnosis and identify needs and situations of vulnerability.
- Monitor individualized work plans.
- Prepare social reports.
- Coordinate with social services, local organizations, and other professionals.
- Participate in the activation of shared spaces and ensure compliance with coexistence regulations.
- Accompany students on practicum placements, where applicable.
- Coordinate and monitor volunteer activities in collaboration with the designated responsible person.
- Record information in digital applications and ensure their proper use.
- Support the work team, where applicable.
- Participate in training activities and professional development opportunities.
- Ensure the proper condition of physical spaces and equipment.
Specific Functions:
1. Welcome and Access to the Program
- Assess referrals and manage pre-admission actions.
- Inform about the international protection system.
- Conduct welcome interviews in coordination with the social educator.
- Prepare program admission documentation and social reports.
2. Individualized Social Intervention and Follow-up
- Develop, monitor, and update individualized work plans (IPI) in coordination with the social educator.
- Identify situations of vulnerability and refer to specialized resources.
- Process economic assistance applications.
3. Comprehensive Accompaniment
- Manage, process, and monitor user needs regarding:
o Housing
o Coverage of basic needs
o Health
o Education
o Social network
4. Resource Management and Coexistence
- Ensure the proper functioning of the resource in terms of welcome, coexistence, and use of spaces.
- Ensure compliance with internal regulations and program protocols.
- Participate in the activation of shared spaces.
- Promote autonomy.
5. Coordination and Network-based Work
- Coordinate with social services, organizations, and public administrations.
- Participate in team meetings and technical coordination spaces.
- Collaborate with the interdisciplinary team in case follow-up, especially with the assigned social educator.
6. Administrative and Technical Management
- Register and update information in the program’s management tools.
- Manage and monitor files in the SIRIA application.
- Prepare social reports and technical documentation.
- Draft program justification reports.
7. Community Action
- Support the development of group activities and community initiatives.
8. Other Duties
- Perform any tasks necessary for the proper functioning of the service.

* Experience: 1 year. Accompaniment of vulnerable groups. Coordination with social services and local organizations. Individualized and family-oriented social intervention.
* Bachelor’s Degree in Social Work.

* Availability for travel.
