Assess the social aspects of patients treated by various units and departments to conduct social diagnoses and implement appropriate interventions for each situation. Counsel, inform, and guide patients requiring such support by informing them about existing and suitable social assistance options for their circumstances. Provide patients or their families with information on available social resources that can address, resolve, or alleviate the issues raised, and offer emotional support where required. Know, manage, and promote access to existing resources among users and professionals from other services who may interact with the patient. Identify local community organization resources to appropriately guide resolution of non-hospitalization cases of a social nature. Update methodologies and techniques through specialized training programs, meetings with other healthcare professionals, and individual or group supervision. Collaborate with the interdisciplinary team by providing essential data from the patient's social history and social diagnosis to jointly design a comprehensive intervention plan, maintaining confidentiality. Facilitate communication between users, their families, and service professionals. Any other responsibilities or duties assigned within the scope of this professional role. * Bachelor's degree in Social Work * Catalan (advanced spoken and written proficiency) * Spanish (advanced spoken and written proficiency) * Competencies / knowledge: Diploma or Bachelor's degree in Social Work; a Master's degree in Healthcare Social Work is desirable; relevant experience in the healthcare sector; user-level computer skills * Permanent employment contract * Part-time (8 hours per week) * Additional details of interest: 20% workload; flexible schedule
