




Job Summary: We are seeking a Social Worker to monitor and attend to the needs of service users and their families, promoting their quality of life at the Calaf Residence. Key Responsibilities: 1. Monitor the needs and quality of life of service users. 2. Develop and evaluate programs for family accommodation and intervention. 3. Collaborate within an interdisciplinary team and with the community. For the Calaf Residence—a center managed by Sant Andreu Salut—we seek a Social Worker to monitor and attend to the needs of service users and their families, promoting their quality of life. \- Establish initial contact with the service user, family, or reference person to identify expectations and align them with the center’s reality. \- Develop, implement, and evaluate the accommodation program for the service user and their family. \- Support the family through various situations or processes experienced by the service user. \- Ensure compliance with confidentiality standards for service users and respect for their right to privacy. \- Develop, implement, and evaluate the family intervention program. \- Intervene in situations involving loss and conflict experienced by the service user, coordinated with the rest of the team. \- Promote participation and ensure all individuals remain informed about cultural and recreational activities that may be developed within the community. Facilitate attendance for those individuals with physical and psychological dependency who cannot attend independently, coordinated with other professionals. \- Develop and monitor the center’s volunteer program, specifically regarding individual social support. \- Inform the service user and/or family, and process, if applicable, matters related to guardianship, advance directives, as well as register and guarantee the final wishes expressed by service users. \- Collaborate with the team in end-of-life accompaniment processes. \- Ensure respect for customs and cultural diversity among service users. \- Carry out administrative procedures and documentation management related to the entire care process of the service user (admission, stay, and discharge). \- In cases where no family or reference person is available, act as the professional reference for the service user. 2. Responsibilities Related to the Interdisciplinary Team: \- Provide other team professionals with information that may contribute to improving comprehensive care for service users. \- Participate in working sessions aimed at developing and monitoring the Care and Life Plan (PAiV), and jointly agree with the service user or responsible person on objectives to be achieved. \- Facilitate communication and relationships between service users, families, and center professionals. \- Participate in developing and periodically reviewing social-area clinical protocols. \- Conduct studies directed toward creating new work methodologies or improving existing ones, aiming to enhance daily practice. \- Participate in preparing the center’s annual activity report and other studies or projects, providing data relevant to the scope of professional competence. \- Coordinate with the rest of the team regarding the annual activity program. \- Collaborate in the continuing education of center professionals, trainees, and volunteers. \- Actively participate, together with other team members, in the center’s continuous improvement plans. 3. Responsibilities Related to the Community: \- Coordinate and collaborate, together with professionals from external institutions, in managing shared social cases and projects. \- Foster interrelation between the center and the community, and promote recognition of the residence and day center services within the community as additional social resources and places of living. \- Jointly intervene with other professionals in community participation bodies. * Minimum 6 months’ experience; prior geriatric experience is valued. * Bachelor’s degree in Social Work. * Catalan (spoken: advanced; written: advanced). * Spanish (spoken: advanced; written: advanced). * Competencies / Knowledge: Communication and persuasion \- Service orientation \- Teamwork and cooperation \- Adaptability to change and flexibility \- Emotional management \- Problem-solving and decision-making ability \- Effectiveness and results achievement \- Planning, organization, and prioritization. * Driver’s license: B. * Permanent employment contract. * Part-time position (15 hours per week). * Additional information of interest: 15 hours per week
