




Accompaniment of workers at the Special Employment Center during their final working stage due to premature aging. The social integrator must carry out the following functions: \- Guidance and support to cope with aging in the most active and healthy way possible. \- Accompaniment through the necessary procedures to implement retirement for individuals with severe mental disorder and premature aging. \- Training activities aimed at reinforcing competencies to help individuals reinterpret and sustain their own life projects during the transition to retirement. \- Individualized assessment of each person’s needs and supportive interventions to facilitate their transition toward full retirement. \- Development of the Individualized Support Plan (PEI) for workers at the Special Employment Center (CET) with severe mental disorder affected by premature aging. \- Joint coordination with Mental Health Teams (EMS) and the rest of the Mental Health Network to support the conclusion of the individual’s working stage across various domains. \- Preparation of the justification report for the Aging Project subsidy. These interventions are complementary to those outlined in the Individualized Strategic Plans developed by the Multidisciplinary Support Teams (EMS) of the CETs. EMS staff must coordinate the Aging Project interventions to ensure person-centered support for CET workers. Experience: 6 months. \- Experience working with persons with disabilities, specifically those with severe mental disorder, is desirable. \- Experience handling procedures related to Social Security pensions is desirable. * Temporary employment contract (6 months) * Full-time working schedule * Gross monthly salary ranging from '1500' to '1600' * Other relevant information: \- Working hours: 7:45 a.m. – 4:15 p.m. \- Immediate start.
