**Objective**

**Functions related to the person we support**
Initial assessment\- welcome
Conduct previously scheduled visits with the person's reference contact.
Identify needs across all areas of their life, whether personal or environmental (e.g., clothing inventory, housing supervision...).
Provide support to foster the development and effective management of their social skills and emotions affecting their socialization areas.
Support the reference contact in accompanying the person we support during procedures such as CAD, dependency assessments, residential transfers, judicial, financial, etc.
Accompany them in group activities scheduled by the organization.
Process and/or assist with the person’s personal documentation (ID card, renewals, transport cards, health cards, etc.).
Purchase personal items, food, clothing, etc. for the person we support.
Record all interventions carried out for the person we support after each activity (procedures, meetings, coordinations, visits...).
Inform the support reference and the center where the person is admitted to improve care and convey general medical information.
Ensure respect for relational customs and cultural diversity of those served.
Provide support and accompaniment to the person we support during various grief processes (death, loss of abilities, housing changes...).
Promote the motivations and interests of the person we support.
Accompany them to medical services, if necessary, and follow up with various healthcare professionals regarding their health.
Accompany them, if necessary, to hospital emergency departments, assessing the situation and determining appropriate actions during working hours.
Provide tools for correct medication administration or health care.
Promote a healthy lifestyle through prevention of sedentary behavior and proper nutrition.
Provide emotional support and guidance to the person supported (PaS), ensuring they receive the human, personal, affective, and relational support they need.
Support the reference contact in managing data protection documentation.
**Functions related to the family**
Accompany the person, if necessary, on family visits when required.
Support the reference contact in informing the family about how the support organization operates.
Jointly assess, together with the reference contact, family relationships or those of collaborating references, and strengthen beneficial relationships.
Support the reference contact in informing the family or closest environment, if the person consents, about changes in the PaS’s personal situation.
Support the reference contact in managing data protection documentation.
**Functions related to the support reference/the team/the organization**
Maintain ongoing communication with the support reference, highlighting aspects most relevant to the person (phone, email...).
Periodically coordinate with team members (meetings between reference and technician).
Collaborate in developing, updating, and monitoring the individualized care plan for the person we support, assessing their needs, choices, and preferences.
Participate in periodic team/organization/resource meetings.
Attend and participate, if necessary, in case supervision sessions.
Manage and justify expenses incurred in carrying out duties.
Report to management any differences in intervention approaches concerning the PaS that may arise within the team.
Consistently pursue improvement regarding the activities we carry out, translating this into suggestions and proposals for improvement addressed to management and coordination.
Organize the schedule of interventions and visits according to what has been agreed upon with the reference contact.
**Functions related to the community or environment**
Support the reference contact in identifying, coordinating, and managing various community-based supports and specialized services across different domains (health, employment or occupational, leisure, social services, external services, housing, other guardianship entities, volunteering...).
Raise awareness and inform the community about guardianship services (functions and competencies).
Foster the person’s integration into the community.
Respect the person’s expressed wishes regarding which relationships they wish to foster and which they do not.
Respect the person’s privacy regarding their community or environment.
**Profile:**
The following will be positively considered:
* Prior experience in the field of disability, preferably in mental health
* Prior experience in guardianship/support organizations
**Mandatory**: Class B driving license
