




*Working hours description:* \- Employment contract for 1,772 annual working hours \- Will carry out the following duties, as well as any others requested that are related to these duties and/or to their academic qualifications or professional competencies: * Provide support during the admission process and prepare the user’s social history and social file. * Collaborate in and conduct studies aimed at investigating the social aspects relating to users. * Carry out administrative tasks and prepare social reports for users, as well as any other reports requested by the center’s management. Provide information about internal and external resources and assess the user’s personal, family, and social situation. * Deliver direct assistance at individual, group, and community levels. * Promote users’ integration and participation in center life and in their surrounding environment. * Participate in preparing and implementing programs to help users adapt to the center. * Coordinate working groups and sociocultural animation activities. * Carry out necessary procedures to resolve social problems affecting users, primarily in coordination with relevant entities and institutions. * Promote coordination with professionals and resources from the social services and healthcare systems to establish communication channels. * Participate, together with the multidisciplinary team or medical department, in developing guidance or care plans tailored to users’ needs. * Participate in assigning and changing users’ rooms and dining tables, in coordination with the nursing department and center management. * Conduct follow-up and, where necessary, visit users in healthcare facilities. * Inform, guide, advise, and accompany users’ families throughout the social care process provided by the center.
