Job Summary: We are seeking a committed and empathetic Social Worker for a senior residence, with responsibilities including processing social benefits, interdisciplinary coordination, and supporting residents and their families. Social Worker Residencia La Serra, located in Ullastrell (Barcelona), is looking to incorporate a qualified Social Work professional who is committed and sensitive toward older adults. Main Responsibilities: - Processing and follow-up of Economic Benefits Linked to Dependency (PEV) and other social assistance programs. - Development, review, and updating of Individual Care Plans (PIA) in coordination with the interdisciplinary team. - Support and guidance for families, accompanying them throughout the admission process and the resident’s stay. - Coordination with social services, Primary Care Centers (CAPs), and other public agencies. - Identification and analysis of social risk situations, making referrals when necessary. - Active participation in interdisciplinary team meetings. - Support during resident admission, follow-up, and discharge processes. Requirements: - University degree in Social Work. - Valid professional registration. - Prior experience in geriatric or socio-healthcare settings (desirable). - Knowledge of the Dependency Law and the functioning of the social services system in Catalonia.
