Their work focuses on direct intervention, facilitating relational spaces, promoting participation, and strengthening support networks, aiming to improve quality of life through strategies tailored to individual and community needs. Functions and Responsibilities 1. Needs Assessment: Conduct individual and group assessments of the social, emotional, and cognitive needs of elderly participants in the Foundation’s programmes to design personalised intervention plans. 2. Programme Implementation: Plan, implement, and coordinate social integration activities for elderly people. 3. Group Intervention: Facilitate group support dynamics, workshops, and group activities that encourage social interaction, personal development, and the creation of support networks among elderly people to combat unwanted loneliness and prevent institutionalisation. 4. Coordination with Other Professionals: Collaborate closely with other professionals—including social workers, psychologists, and socio-healthcare staff—to ensure a comprehensive intervention approach with elderly people. 5. Promotion of Autonomy: Foster the independence of elderly people by providing them with the necessary tools and support to make informed decisions about their lives and environment. 6. Project Management: Participate in the management and evaluation of the Foundation’s social inclusion projects, ensuring achievement of established objectives and proposing improvements to intervention processes.
