We are seeking personnel to provide home-based support, focused on monitoring, accompanying, and implementing individualized care plans. The primary responsibility is to deliver comprehensive care to individuals requiring assistance during acute illness or deterioration of health, covering Activities of Daily Living (ADLs). Assistance is also provided in instrumental and social activities, supporting each patient's autonomy and well-being. Additionally, staff participate in programs aimed at restoring personal autonomy and in managing the patient\-nurse alarm system, ensuring a rapid response to any emergency. This role requires commitment to quality of care, empathy, and respect, adapting to each person's specific needs and fostering a safe and trustworthy environment within their own home. The ability to work autonomously and coordinate with other healthcare professionals, if necessary, while maintaining fluent and professional communication, is highly valued. **Required qualifications include one of the following:** Vocational Training Certificate (FPGS) in Nursing Auxiliary Technician; Vocational Training Certificate (FPGS) in Care for Dependent Persons; Professional Certification in Socio-Healthcare for Dependent Persons in Social Institutions; or Professional Certification in Socio-Healthcare for Dependent Persons at Home. Medium-level Vocational Training qualification. Knowledge of nursing auxiliary practices.
