Responsibilities:
* Functional assessment and individualized care planning for each resident.
* Collaboration in designing and developing rehabilitation programs and activities to be implemented at the Mini-Residence and the Apartment.
* Assessment and individualized care planning.
* Implementation—jointly with educators and occupational therapists—of programs and activities carried out at the Mini-Residence and the Apartment.
* Coordination with community resources and user follow-up.
* Coordination with various services within the Mental Health Care Network.
* Collaboration in internal programming and operational organization of the Mini-Residence and the Apartment.
* Collaboration in evaluating the care process for each resident, as well as the overall functioning of the Mini-Residence and the Apartment.
* Collaboration in coordination with Mental Health Services.
* Collaboration in the center’s overall evaluation.
