**Position at a day center for elderly individuals**
Work is carried out at a day center focused on caring for elderly individuals, where social and support services are provided. The selected candidate will conduct comprehensive social assessments of users to understand their needs, resources, and personal circumstances. Based on these assessments, individualized social intervention plans will be designed, including specific objectives and actions aimed at improving users' well-being and autonomy. This work is conducted collaboratively with other professionals at the center, such as healthcare staff and caregivers, to ensure coordinated care.
Information and counseling are also provided to both users and their families. They are informed about available social resources, eligible benefits, and how to apply for them. Additionally, daily support is offered to family caregivers, helping them manage stress and difficulties that arise. The position involves coordinating with external services—such as municipal social services or neighborhood organizations—to build a robust support network.
Another important aspect of the role is maintaining up-to-date social and administrative documentation, including detailed reports on each user’s progress. Efforts are made to encourage elderly individuals’ participation in center activities and in their broader community, promoting their integration and quality of life. All of this is done from a person-centered approach, grounded in ethical and professional commitment.
