




Job Summary: Health promotion, comprehensive user assessment, medication administration and monitoring, coordination with medical services and families, and clinical record management. Coordinate with CAP, Hospital, and specialized services as needed. Take and monitor vital signs, and perform blood draws. Coordinate with pharmacy according to medication modifications and/or prescriptions. Perform nursing care. Manage clinical records and files. Ensure proper hygiene of users and facilities. Maintain ongoing communication with users' families. Design and actively participate in the Individualized Care Plan (PAI). Coordinate with the interdisciplinary team for comprehensive user care.
