SUPPORT TECHNICIAN (OSONA)

Description

We are seeking a Support Technician for our Legal Capacity Support Service at Fundació Vella Terra, serving the Osona region (Vic). User profiles include mental health conditions, disabilities, behavioral disorders, elderly individuals, etc. Main responsibilities * Conduct periodic and follow-up home visits and community-based visits to service users. * Provide individualized accompaniment in daily tasks, medical appointments, and administrative or legal procedures. * Support planning and management of daily life, promoting autonomy and active participation. * Collaborate in the development, implementation, and monitoring of the Individualized Action Plan (PAIV), tailored to users’ needs and preferences. * Identify and intervene in situations of vulnerability or risk of abuse, applying protection protocols. * Facilitate communication and mediation with family members, social networks, and professionals. * Maintain documentary records and keep case files updated. * Participate in interdisciplinary team meetings and coordinate with the network of available resources. Valued professional experience * Intervention with individuals experiencing mental health disorders in community and residential settings. * Accompaniment of individuals diagnosed with mental health conditions. * Accompaniment of dependent elderly individuals. * Work in the field of drug addiction and addictive behaviors (high-demand programs/harm reduction). * Support for individuals with intellectual and/or physical disabilities. * Care and accompaniment of individuals under legal capacity support measures. * Required competencies * Empathy, active listening, and assertive communication. * Planning, organization, and efficient task and time management. * Social needs analysis and personalized intervention. * Network-based work and interdisciplinary coordination. * Crisis management and conflict resolution. * Professional autonomy and responsibility. Requirements: \- Vocational Training Cycle in Social Integration \- Experience in Decision-Making Support services. \- Experience in direct care for individuals diagnosed with mental health conditions or elderly individuals. \- Knowledge of mental health, disability, and aging. \- Valid driver’s license and driving experience. \- Strong motivation to learn.
