Support people with intellectual disability and mental illness in their daily lives and their autonomy process, working in network collaboration and familiarizing oneself with community resources.

**Main Responsibilities:**
* Supporting people with intellectual disability and mental illness in daily life, working within the community.
* Accompanying individuals throughout their autonomy process and developing a personalized care plan for each person served.
* Working in network collaboration, coordinating\-with various stakeholders and services, and identifying community resources.

**Requirements:**
* Mandatory: Higher Vocational Training Certificate (CFGS) in Social Integration.
* Experience working with people with intellectual disability and mental illness will be valued.
* We seek a dynamic, responsible individual with flexible working hours.
* Valid driver’s license and personal vehicle required.
**Offered:** 1 permanent position at 50% full-time equivalent
**Working Hours**: Afternoons and some mornings (to be specified during the interview)
