We stand out for two important lines of activity: home care for patients with complex chronic pathologies and medical emergencies in the home. To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients. **How will you CONTRIBUTE and GROW?** Perform professional duties related to home care for patients covered under the Emergency Contract in the Basque Country. Collaborate in meeting quality, environment, and safety procedures across various business units. **Anamnesis:** The physician collects data from the patient and/or caregivers regarding signs and symptoms of their illness, medical history, review of available reports, allergies, etc. **Physical examination:** A physical examination will be performed, including assessment of the latest diagnostic tests available at the patient's home, nutritional status, and psychological state of the patient. **Nursing assessment:** Based on all data gathered during anamnesis and clinical examination, the physician performs a comprehensive evaluation of the patient. **Medical Diagnosis:** If the information obtained is conclusive, the physician issues a diagnosis and codes it according to the CIE10 code list defined by the client. If additional tests not available at home are required, the physician will request patient referral to confirm the potential diagnosis. **Treatment and/or care plan:** The prescribed treatment and/or care plan will be thoroughly explained to the patient and/or caregivers and documented in the clinical record report. **Discharge at home/Discharge with referral:** At the end of the medical intervention, the physician will provide discharge at home or, if necessary, refer the patient, in coordination with the emergency coordination center, to the reference hospital.
