We stand out in two key areas: home care for patients with complex chronic conditions and home medical emergencies To this end, we have a team of proactive and dynamic healthcare professionals committed to preserving the lives of our patients **How can you CONTRIBUTE and GROW with us?** Perform professional duties related to home care for patients covered under the Emergency Contract in the Basque Country. Collaborate in complying with quality, environmental, and safety procedures across various business units. Anamnesis: the physician collects data from the patient and/or caregivers regarding symptoms and signs of their illness, medical history, review of available reports, allergies, etc. Physical examination: a physical examination will be conducted, including assessment of the latest diagnostic tests available at home, nutritional status, and psychological state of the patient. Nursing assessment: using all data obtained from both anamnesis and clinical examination, the physician performs a comprehensive patient evaluation. Medical Diagnosis: if the information gathered is conclusive, the physician issues a diagnosis and codes it according to the CIE10 code list defined by the client. If additional tests not available at home are required, the physician will request patient referral to confirm the potential diagnosis. Treatment and/or care plan: the prescribed treatment and/or care plan will be thoroughly explained to the patient and/or caregivers and recorded in the medical record report. Discharge at home/Discharge with referral: upon completion of the medical intervention, the physician will discharge the patient at home or, if necessary, refer them to the reference hospital in coordination with the emergency coordination center.
