Job Summary: This role involves providing home medical care for patients with complex chronic conditions and medical emergencies, focusing on anamnesis, physical examination, nursing assessment, diagnosis, treatment, and patient discharge. To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients. **How will you CONTRIBUTE and GROW?** Perform duties inherent to your profession in providing domiciliary care to patients covered under the Basque Country Emergency Contract. Collaborate in implementing quality, environmental, and safety procedures across various business units. **Anamnesis:** The physician collects data from the patient and/or caregivers regarding signs and symptoms experienced by the patient, clinical history, analysis of available reports, allergies, etc. **Physical Examination:** A physical examination will be performed, along with evaluation of the most recent diagnostic tests available at the patient’s home, nutritional status, and psychological status of the patient. **Nursing Assessment:** Based on all data obtained during both anamnesis and clinical examination, the physician conducts a comprehensive assessment of the patient. **Medical Diagnosis:** If the information gathered is conclusive, the physician issues a diagnosis and codes it according to the CIE-10 code list defined by the client. If additional diagnostic tests not available at home are required, the physician will request referral of the patient to confirm the possible diagnosis. **Treatment and/or Care Plan:** The treatment regimen and/or care plan will be explained in detail to the patient and/or caregivers and documented in the clinical record report. **Home Discharge/Discharge with Referral:** Upon completion of the medical intervention, the physician will discharge the patient at home; if deemed necessary, the physician will refer the patient—according to coordination with the emergency center—to the designated reference hospital.
