




Job Summary: This role involves providing home-based medical care for emergency patients, focusing on diagnosis, treatment, and patient well-being. Key Responsibilities: 1. To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients. **How will you CONTRIBUTE and GROW?** Perform professional duties related to home-based medical care for patients covered under the Basque Country Emergency Contract. Collaborate in implementing quality, environmental, and safety procedures across various business units. **Medical History Taking:** The physician collects information from the patient and/or caregivers regarding signs and symptoms of their illness, clinical history, analysis of available reports, allergies, etc. **Physical Examination:** A physical examination is performed, including assessment of the most recent diagnostic tests available at the patient’s home, nutritional status, and psychological status. **Nursing Assessment:** Based on all data obtained during both medical history taking and clinical examination, the physician performs a comprehensive assessment of the patient. **Medical Diagnosis:** If the collected information is conclusive, the physician issues a diagnosis and codes it according to the CIE-10 code list defined by the client. If additional diagnostic tests not available at home are required, the physician will request referral of the patient to confirm the potential diagnosis. **Treatment and/or Care Plan:** The treatment regimen and/or care plan will be thoroughly explained to the patient and/or caregivers and documented in the clinical record. **Discharge at Home/Referral Discharge:** Upon completion of the medical intervention, the physician discharges the patient at home; if deemed necessary, the physician refers the patient—according to coordination by the Emergency Coordination Center—to the designated reference hospital.
