




Job Summary: Nurse for home-based care of patients with complex chronic conditions and emergencies, performing anamnesis, physical examination, nursing assessment, and nursing procedures. We stand out for two important lines of activity: home care for patients with complex chronic pathologies and medical emergencies in the home. To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients. **How will you CONTRIBUTE and GROW?** Perform duties inherent to your profession in home-based care for patients covered under the Basque Country emergency contract. Anamnesis: The nurse collects data from the patient and/or caregivers regarding signs and symptoms of their illness, clinical history, analysis of available reports, allergies, etc. Physical examination: A physical examination, nutritional status assessment, and psychological status assessment of the patient will be conducted. Nursing assessment: Using all data gathered during both anamnesis and physical examination, the nurse performs the patient assessment. Nursing procedures: Required nursing procedures will be performed, including wound dressings, suturing, catheterization, etc. If a specific care plan is required until another healthcare resource within the system intervenes, the nurse—when deemed appropriate—will define such a care plan, explain it in detail to the patient and/or caregivers, and document it in the clinical record. Case registration in the clinical record. Home discharge/Home discharge with referral: Upon completion of the intervention, the nurse concludes the intervention at the patient’s home; if necessary, the nurse refers the patient—per coordination with the emergency coordination center—to a hospital.
