




Job Summary: We are seeking a Social Worker for social intervention and follow-up, working in coordination with teams and services to promote personal autonomy. Key Responsibilities: 1. Our goal is to create nearby, humane, and coordinated care environments, integrated with the community and various local social actors. We are currently seeking to temporarily incorporate a **Social Worker** to cover the vacation period of the social work team across various resources and services of the organization. The selected candidate will carry out social intervention and follow-up tasks, working in coordination with the rest of the professional team and Local Social Services. Main responsibilities include: * Conducting home visits—both initial introductory visits and follow-up visits—to assess the situation and needs of service users and their environment. * Preparing social reports and recording information related to intervention and follow-up processes. * Ongoing coordination with Local Social Services and other community actors to ensure comprehensive, situation-appropriate support. * Managing and monitoring incidents, conflicts, and complaints, facilitating mediation, communication, and solution-seeking. * Providing social accompaniment and guidance to service users and their families, promoting close, personalized, person-centered support. * Participating in case follow-up and internal coordination with various Aztertzen teams and services. Essential Requirements: * Bachelor’s degree or Diploma in Social Work. * C1 level proficiency in Basque (EGA or equivalent). * Valid driver’s license and personal vehicle. **Particularly Valued** * Prior experience in social intervention, home-based care, or community work. * Strong organizational skills and autonomy in managing daily work. * Interpersonal, communication, and teamwork skills. * Social sensitivity, person orientation, and ability to manage complex situations.
