We stand out for two important lines of activity: home care for patients with complex chronic pathologies and medical emergencies in the home. To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients. **How will you CONTRIBUTE and GROW?** Perform duties inherent to your profession in home-based care for patients covered under the Basque Country emergency contract. Anamnesis: The nurse collects data from the patient and/or caregivers regarding signs and symptoms experienced due to their illness, clinical history, analysis of available reports, allergies, etc. Physical examination: A physical examination, nutritional status assessment, and psychological status assessment of the patient will be conducted. Nursing assessment: Based on all data gathered during anamnesis and physical examination, the nurse performs a comprehensive patient assessment. Performance of nursing procedures: Required nursing procedures will be carried out, including wound dressings, sutures, catheterizations, etc. In cases requiring a specific care plan until assistance from another healthcare resource within the system is provided, the nurse—should they deem it appropriate—will define a detailed care plan, clearly explaining it to the patient and/or caregivers and documenting it in the clinical record. Case registration in the clinical record. Home discharge / Discharge with referral: Upon completion of the intervention, the nurse concludes the intervention at the patient’s home; if deemed necessary, the nurse refers the patient—per coordination with the emergency coordination center—to the hospital.
