The Catalan Down Syndrome Foundation promotes and protects respect for the inherent dignity and equality of rights, freedoms, and opportunities of all individuals with Down syndrome or other forms of intellectual disability. We are seeking a person to join the Independent Living Service "I'm Moving Home" (SVI), accompanying adults with Down syndrome or other forms of intellectual disability in their journey toward autonomy and independent living, in alignment with the Foundation’s mission, vision, and values. Functions and responsibilities: • Accompany service users in developing daily living skills (nutrition, hygiene, household and community environment management). • Provide support, mediation, and cognitive accessibility to facilitate autonomous decision-making by service users. • Participate in monitoring and implementing the Personalized Care Plan (PAP), contributing relevant information to professional reference staff. • Coordinate with families and the professional team to ensure consistency of support and quality of care. • Manage documentation, records, and communications related to the service. • Ensure ethical accompaniment and high-quality interaction with service users and their families. * Experience: 6 months. More than 6 months’ experience in similar roles. * Higher Vocational Training (FP) qualification. * Catalan (spoken: intermediate; written: intermediate). * Spanish (spoken: intermediate; written: intermediate). * Competencies / knowledge: • Empathy and interpersonal communication skills. • Flexibility and adaptability to change. • Ability to plan and organize work. • Ability to solve problems and manage urgent situations. • Teamwork and effective communication. • Autonomy and sense of responsibility.
