




Job Summary: We are seeking personnel for home-based support, focused on comprehensive care and follow-up of individualized care plans for individuals with acute conditions or deteriorating health, promoting their autonomy. Key Highlights: 1. Comprehensive care and follow-up of individualized care plans. 2. Promotion of patient autonomy and well-being at home. 3. Management of patient-nurse alarm systems and rapid response. We seek personnel to provide in-home support, focusing on the follow-up, accompaniment, and implementation of individualized care plans. The primary responsibility is to deliver comprehensive care to individuals requiring assistance during acute pathological processes or health deterioration, covering Activities of Daily Living (ADLs). Assistance is also provided in instrumental and social activities, supporting each patient’s autonomy and well-being. Additionally, staff participate in programs aimed at recovering personal autonomy and in managing the patient\-nurse alarm system, ensuring a rapid response to any emergency. This role requires commitment to quality of care, empathy, and respect, adapting to each person’s specific needs and fostering a safe and trusting environment within their own home. The ability to work autonomously and coordinate with other healthcare professionals—when necessary—while maintaining fluent and professional communication is highly valued. **Required qualifications include one of the following:** Vocational Training Certificate (FPGS) in Nursing Auxiliary Technician; FPGS Certificate in Care for Dependent Persons; Professional Certification in Sociohealthcare for Dependent Persons in Social Institutions; or Professional Certification in Sociohealthcare for Persons at Home. Medium-level Vocational Training qualification. Knowledge of nursing assistance.
