* First reception, assessment and social diagnosis of individuals linked to the entity. * Accompaniment as reference persons for those served in the development and execution of their independent living processes. * Recording of social follow-up and intervention-related data in corresponding databases and computer programs. * Internal coordination with other programs and services of the entity to ensure comprehensive and interconnected support across different areas of the entity. * Facilitation of group social work activities. * Networking to promote the entity and foster coordinated action and response among various social and health network agents related to the entity. * Participation in meetings, committees, and working groups within various public and private organizations. * Analysis and monitoring of indicators for different programs and projects within the department. * Involvement in the design, evaluation, and planning of actions aimed at better identifying the needs of people with disabilities supported by the department. * Participation in the design, evaluation, and planning of innovative initiatives to engage individuals linked to the entity, such as training sessions, events, informational meetings, among others. **REQUIREMENTS** * Diploma or Degree in Social Work * Minimum of 1 year of experience in a similar position * Proficiency in Catalan and Spanish. Knowledge of other languages (English, French, Arabic) is an asset. * Proficient in Microsoft Office and technological management tools. * Knowledge of social sector databases is valued. * Class B driver's license with access to a vehicle: motorcycle or car. * Strong communication and interpersonal skills.
