Position Summary: A qualified professional responsible for conducting comprehensive functional assessments and coordinating quality-of-life and support-needs assessments. Key Responsibilities: 1. Conduct physical and cognitive functional assessments. 2. Coordinate quality-of-life and support-needs assessments. 3. Collaborate as part of a team and advise on assistive technology. We seek a qualified professional capable of performing comprehensive functional assessments of individuals, covering physical and cognitive aspects, as well as their autonomy in daily activities and use of assistive technology. The role involves coordinating quality-of-life and support-needs assessments and actively participating in the follow-up of Individual Support Plans. The candidate will collaborate with other professionals to support individuals with intellectual or developmental disabilities, advising on the use of assistive technology and identifying community resources. Responsibilities include developing and implementing health and prevention programs, facilitating social reintegration, and introducing Alternative and Augmentative Communication (AAC) systems. Flexibility to work across diverse settings and documentation of performed activities are required. A full-time, temporary contract for 10 months is offered.
