Position Summary: The Germà Tomàs Canet Foundation is seeking a Support Measures Program Coordinator to accompany vulnerable individuals and promote their personal autonomy and social integration. Integration and full participation in social life The Germà Tomàs Canet Foundation, part of the Hospitaller Order of Saint John of God, is a non-profit organization whose mission is to support people in situations of vulnerability and at risk of social exclusion through the management of programs and services aimed at improving quality of life and social integration. Currently, the Foundation is seeking a Support Measures Program Coordinator at its Sant Boi de Llobregat office to ensure appropriate care for protected individuals, promote their personal autonomy, holistic development, recovery of capacity, and full incorporation, integration, and participation across all areas of social life—always aligned with the values and working methodology of the Hospitaller Order of Saint John of God. Responsibilities Regarding the Person Supported: - Conduct initial and ongoing assessments of the person supported. - Welcome the person supported, inform\-them, and orient\-them regarding the Support Measures Service. - Identify personal needs across all domains and develop strategies and alternatives to maintain and improve the person’s quality of life. - Foster the relationship between the person supported and the Support Measures Assistant. - Develop, update, and monitor the Individual Care Plan jointly with the interdisciplinary team, taking into account the person’s needs, choices, and preferences. - Provide support and accompaniment to the person supported during various grief processes (e.g., death, loss of capacities, housing loss, etc.). - Establish, together with the economic department, the financial budget and prioritize\-expenditures, as well as monitor and control spending. - Prepare relevant reports. - Conduct periodic visits to the person supported. - Assess authorizations required for the various activities carried out by the person supported. - Accompany the person supported in situations requiring such support (e.g., activities, medical appointments, transfers between centers, etc.). - Conduct the initial social study of subsidy modules and provide information needed to update\-them. - Work toward establishing a strong relationship between the person supported and the Coordinator. Responsibilities Regarding the Family: - Inform the family about the Coordinator’s role and that of the organization. - Assess family relationships and/or relationships with other reference persons, and strengthen beneficial connections. - Hold periodic meetings, as appropriate, with families connected to the person supported. - Inform the family or closest environment, as appropriate, about changes in the person’s personal situation. Responsibilities Regarding the Community and/or Environment: - Identify, coordinate, and manage various community-based supports and specialized services across different domains (health, employment/occupational, leisure, social services, external services, housing, other support entities, and volunteering, etc.). - Promote the person’s integration into the community. - Raise awareness and inform the community about the service (functions, competencies). - Jointly welcome and accompany students on practice placements within the program, as applicable. - Carry out occasional travel to other offices/facilities, as applicable. Contract type: Temporary (maternity leave cover) Salary range: €26\.007,66 gross per year (12 payments) Start date: Immediate
