The Addiction Department is recruiting for its **Rochefort site (17)** a **Social Coordinator** H/F on a **1-month fixed-term contract**, part-time 32 hours per week **Responsibilities:** Under the supervision of the department head, your mission includes welcoming, accompanying and/or referring patients and their families internally or externally. The Addiction Care Pole is recruiting for its **Rochefort site (17)**, a **State Registered Nurse (F/M)** on a full-time 2-month fixed-term contract. **Responsibilities:** Under the supervision of the department head, your main duties include welcoming, assessing, supporting, and/or referring patients and their families internally or externally. Other responsibilities related to the position: * Implement actions within the framework of the patient’s overall care plan. Collaborate closely with the multidisciplinary team at the site (doctors, psychologists, social workers) * Support individuals in becoming active participants in shaping their life paths * Emphasize harm reduction and risk mitigation in patient support * Collaborate with external stakeholders involved in the individual's environment * Carry out preventive actions at both individual and collective levels based on demand analysis, aimed at various target groups * Offer clinical alternatives to traditional care (various workshops, discussion groups, etc.) * Conduct nursing follow-up interviews on-site and off-site (interventions in prisons, partner organizations' premises) * Complete digital records and statistical entries.
