Social Worker – Full-Time Position At **INIMA Rehabilitation**, we are seeking to incorporate a **Social Worker** with experience in the sociosanitary field into our team—someone who wishes to be part of a project focused on improving the quality of life of individuals and their families within their own environment. We primarily work with elderly individuals, people in situations of dependency, and patients with neurological conditions, providing comprehensive and coordinated home-based care. What are we looking for? We seek a candidate with: * A Bachelor’s degree or Diploma in Social Work. * Prior experience in the **sociosanitary field, dependency, disability, or elderly care**. * Knowledge of social resources, benefits, support programs, and services related to dependency. * Ability to conduct social assessments and identify needs of both service users and their family environments. * Strong communication, active listening, and family support skills. * Capacity to collaborate effectively with other healthcare and sociosanitary professionals. * Organizational skills, autonomy, and sensitivity when interacting with individuals in situations of heightened vulnerability. What will your responsibilities be? Your main responsibilities will include: * Conducting social assessments of patients and their families. * Providing guidance regarding available resources, benefits, support programs, and services. * Supporting families through processes related to dependency, disability, and social need situations. * Identifying social needs that may affect the individual’s well-being and clinical progression. * Coordinating interventions with family members, primary caregivers, and other professionals. * Collaborating closely with INIMA’s multidisciplinary team to deliver truly comprehensive care. * Monitoring cases and supporting families throughout the entire care process.
